Imperial County Community Health Assessment and Community Health Improvement Plan
The Imperial County Community Health Improvement Partnership joined in a collaborative effort to assess and develop the 2024–2027 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). The goal of the CHA is to evaluate the current health status of the community and assess health risk factors and outcomes. The CHIP is a three-year collaborative effort designed to address the health challenges identified in the CHA.
A special thank you to community members, community-based organizations, and health and governmental agencies for their collective efforts to improve health in our community.
We'd like to invite you to explore key findings from the 2024 Community Health Assessment, highlighted in the Taking Action document. Complete assessment findings can be found in the full Community Health Assessment document. The CHIP document outlines how priority areas identified in the CHA will be collaboratively addressed over the next three years.
Letter to the Community
The Imperial County Community Health Partnership believes that health and well-being go beyond the doctor's office or hospital to the spaces in our community in which we live, work, learn, and play. It is with great pride that we present this summary of the Imperial County 2024 Community Health Assessment (CHA).
Our goal is for the CHA to help make positive changes in the county so that everyone has access to the essential services and supports they need to thrive. Health Priorities
The Imperial County Health Priorities for the 2024 – 2027 Community Health Improvement Plan are:
Priority Area #1: Access to High-Quality Healthcare.
- Improving access to primary care and specialty care providers,
- Improving access to mental health providers and substance use disorder treatment, and
- Improving the number of residents who engage in the core set of preventive healthcare services.
Priority Area #2, Healthy and Safe Living.
- Improving access to healthy and nutritious foods,
- Improving access to affordable and safe housing, and
- Improving environmental and economic factors that contribute to poor health outcomes.
Priority Area #3, Behavioral Health.
- Reduce the prevalence of substance misuse and abuse,
- Improve access to mental health services, and
- Improve the mental health status of young people.
